Vaccine Clinic for Washington Township Residents

Community Vaccine Clinic 

At: Washington Township Community Center 

35 E Springtown Rd, Long Valley, NJ 07853  

On the following dates:

Tues Sept 29^th from 2:00- 6:00 PM

Tues Oct 20^th from 2:30 – 6:00 PM

The following vaccines will be available: 

1. Flu vaccines for ages 5 years and older
2. Senior Flu Vaccines for ages 65 and older
3. Pfizer COVID vaccine for ages 12+
4. Moderna COVID vaccine for ages 12+
5. PCV20/21 Pneumonia Vaccines. ONE TIME DOSE is recommended for those age 50 and older
6. TDAP (Adult Tetanus Booster) for ages 11 and older 

Reservations are encouraged, and additional vaccines will be available for walk-ins.  

Vaccines are free under most insurance plans.

However, we regret to inform you that Green Brook Family Medicine does not accept the following insurance plans:

1. All HMO’s
2. NJ Family Care /Medicaid Insurance.
3. Magnacare/OSCAR/PHCS/Clover Insurances
4. Dual Plan Horizon Medicare Advantage
5. United Health Care/Oxford/UMR Commercial Insurance plans.  
6. Centivo Insurance plans.

If your insurance is not one we accept and you prefer to pay in cash, the cost will be as follows.

Reg Flu             $40.00            Senior Flu        $80.00         COVID  $140.00

TDAP               $50.00

On the day of the clinic:

1. Please bring all insurance cards to the clinic to determine eligibility.
2. A parent/guardian is required to be present with children under the age of 18.
3. A provider from Green Brook Family Medicine will be available at the clinic to address any vaccine related questions.
