Seasonal Flu Shots Available

WASHINGTON TOWNSHIP, MORRIS COUNTY
ANNUAL FLU SHOTS, RABIES CLINIC, AND
SMAC BLOOD SCREENING EVENTS

Flu Shots –
DATE: Wednesday, October 20, 2010;
LOCATION: Washington Township Municipal Building, 43 Schooley’s Mountain Rd., Long Valley (main meeting/court room), NJ:
TIME: 4:30 pm to 7:30 pm OR while supplies last. Please do not arrive early;
FREE only to Residents with Medicare Part B; and AVAILABLE at a FEE of $20.00 to anyone over 18 years old. 
NOTE: Pregnant women must have a doctor’s note. 

REGISTRATION: ONLY for Residents with Medicare Part B is required and these residents must come into the Washington Township Health Department at the Municipal Building beginning September 20, 2010 to pre-register. 

FREE Rabies Clinic for Dogs and Cats for ALL NJ Residents – DATE: Saturday, November 6, 2010; LOCATION: Washington Township Dept. of Public Works facility, 54 Rock Road, Long Valley, NJ; TIME: 9:00 am to 11:00 am OR while supplies last.  Dogs must be leashed and cats must be crated.

SMAC Blood Screening – DATE: Saturday, November 13, 2010; LOCATION: Long Valley First Aid Squad, 70 East Mill Rd., Long Valley, NJ; TIME: 8:00 am to 11:00 am; and FEE: $25.00 and PSA testing offered for an additional $25.00; REGISTRATION: pre-registration is required before November 10, 2010 and residents must come into the Washington Township Health Department to pre-register.
